Despite advances in transplantation medicine, early detection of allograft injury remains a challenge. The current diagnostic gold standard—biopsy—is invasive, subject to variability in the interpretation of histologic changes and often not representative of the entire graft status. This underscores the growing need for reliable, non-invasive biomarkers that enable timely detection of rejection and more effective monitoring of allograft function. Extracellular vesicles (EVs), small membrane-bound particles released by cells to mediate intercellular communication, are emerging as promising biomarkers. Their molecular cargo reflects the physiological state and origin of the parent cells, and they are present in various body fluids, including urine - a readily available and non-invasive sample. Urinary extracellular vesicles (uEVs) therefore offer a compelling opportunity for real-time insight into the status of the transplanted kidney. This review highlights the diagnostic potential of uEVs in kidney transplantation, focusing on their biogenesis, isolation methods, molecular characteristics, and their emerging role in the early detection of allograft rejection</dc:description><dc:description xml:lang="sl">Presaditev ledvice je najuspešnejša oblika zdravljenja končne ledvične odpovedi, saj zagotavlja boljšo kakovost življenja in daljše preživetje bolnikov kot dializa. Kljub napredku ostaja zgodnje odkrivanje okvare presadka zahtevno, saj je biopsija – trenutni diagnostični standard – invazivna, podvržena subjektivni presoji histopatoloških sprememb in pogosto nereprezentativna za stanje celotnega presajenega organa. Zato obstaja potreba po zanesljivih, neinvazivnih bioloških označevalcih, ki bi omogočili pravočasno odkrivanje okvare in učinkovitejše spremljanje delovanja presadka. Zunajcelični vezikli (ZV), majhne membranske strukture, ki jih celice sproščajo za medcelično komunikacijo, izkazujejo velik potencial kot biološki označevalci. Njihova sestava odraža stanje starševskih celic, nahajajo pa se v različnih telesnih tekočinah, tudi v urinu, ki je enostavno dostopen in neinvaziven za zbiranje. Urinski ZV (uZV) tako predstavljajo obetavno diagnostično orodje za neinvazivno spremljanje presajenih ledvic.
