Pain above the level of SCI is usually caused by trauma or overuse and is nociceptive; pain at or below the level of SCI is a consequence of complex central mechanisms and is neuropathic. Treatment of chronic pain in persons with SCI is seldom successful. Chronic pain affects quality of life. Methods: The Medline database, Cochrane Database of Systematic Reviews, EBSCOhost and PEDro databases were systematically searched using multiple search terms related to pain and functioning. Results: Neuropathic pain in SCI is the consequence of a complex combination of peripheral and central neural system mechanisms and is influenced by multiple factors. There is no drug that would constantly diminish chronic neuropathic pain in persons with SCI, so they have to learn how to cope with pain. Diminishing the pain is the most frequently unfulfilled request of persons with SCI. The bio-psycho-social-model-based approach should aim to improve one's capabilities of coping with pain and thereby improving quality of life. Conclusions: To select the most appropriate treatment, the use of classification instruments and methods is necessary in clinical practice. Psychosocial profile needs to be part of a multidisciplinary protocol of treatment of pain in persons with SCI</dc:description><dc:description xml:lang="sl">Izhodišča: Osebe z okvaro hrbtenjače (OH) pogosto občutijo bolečine, ki so nad ravnijo okvare hrbtenjače najpogosteje posledica poškodbe ali obrabe in so po mehanizmu nastanka nociceptivne, na ravni okvare hrbtenjače ali pod njo pa so največkrat posledica kompleksnih centralnih mehanizmov in so nevropatske. Zdravljenje kronične bolečine pri osebah z OH je večinoma manj uspešno. Kronična bolečina vpliva na kakovost njihovega življenja. Metode: Sistematično smo pregledali literaturo o povezanosti med bolečino in funkcioniranjem oseb z OH v podatkovnih zbirkah: Medline, Cochranovi podatkovni zbirki sistematičnih pregledov, EBSCOhost in PEDro za vsa dosegljiva leta do 2012. Rezultati: Nevropatska bolečina po OH je posledica kompleksnih perifernih in centralnih mehanizmov pri okvarah živčevja. Na njen razvoj vpliva več dejavnikov. Doslej še ni zdravila, ki bi pri osebah z OH stalno zmanjševalo kronično nevropatsko bolečino, zato se mora večina, žal, naučiti živeti z njo. Zmanjšanje bolečine je tista zahteva oz. želja oseb z OH, ki najpogosteje ni uresničena. Pristop k obravnavi bolečine, ki temelji nabio-psihosocialnem modelu, mora biti usmerjen k izboljšanju zmožnosti posameznika za življenje z bolečino in tako tudi k izboljšanju kakovosti njegovega življenja. Zaključki: V klinični praksi moramo uporabljati metode in orodja za klasifikacijo bolečine, da bi glede na to lahko izbrali najbolj primerno metodo zdravljenja.
