There are several functional tests available to assess the adrenal cortex function: rapid and multi-day ACTH tests, insulin tolerance test, metyrapone test and dexamethasone tests. In the rapid ACTH test, cortisol secretion from the adrenal glands is stimulated by injecting tetracosactide. The resulting cortisol level is compared to the baseline ACTH level before the test. If the cortisol level does not increase 30– 60 minutes after the application, it indicates adrenal cortical atrophy. In Slovenian hospitals and outpatient clinics, tetracosactide solution with a concentration of 5 µg/mL is most often prepared in a glass vial. In 2016, the Central Pharmacy of the University Medical Centre Maribor started with the aseptic preparation of tetracosactide solution filled in syringes at the request of hospital departments. The industrial drug was diluted in sodium chloride solution under aseptic conditions. To perform a rapid ACTH test, it is sufficient to load the patient with 1 µg of tetracosactide. The article describes a pharmacopoeial method for the evaluation of tetracosactide and the use of stateof-the-art analytical methods that could be used to evaluate tetracosactide final preparations</dc:description><dc:description xml:lang="sl">Tetrakozaktid je sintezni analog človeku lastnega adrenokortikotropnega hormona (ACTH), ki spodbuja sproščanje kortizola iz skorje nadledvične žleze. Na voljo imamo več funkcijskih testov za oceno delovanja skorje nadledvične žleze: hitri in večdnevni test ACTH, inzulinski tolerančni test, metiraponski test in deksametazonske teste. Pri hitrem testu ACTH z injiciranjem tetrakozaktida sprožimo izločanje kortizola iz nadledvičnih žlez. Tako pridobljeno raven kortizola primerjamo s tisto pred testom – bazalni ACTH. Če koncentracija kortizola 30 do 60 minut po aplikaciji ne naraste, je skorja nadledvičnih žlez atrofična. V slovenskih bolnišnicah in ambulantah največkrat pripravljamo raztopino tetrakozaktida s koncentracijo 5 µg/mL v steklenih vialah. V Centralni lekarni Univerzitetnega kliničnega centra Maribor (UKC Maribor) že od leta 2016 za potrebe oddelkov polnimo raztopino tetrakozaktida v brizge. Redčenje industrijskega zdravila s fiziološko raztopino opravimo po aseptičnem postopku. Za izvedbo hitrega testa ACTH zadostuje obremenitev preiskovanca z 1 µg tetrakozaktida.
