After evolving from coma, some of them show no evidence of awareness of self and environment and give no meaningful response for the external stimuli; such condition is called a vegetative state. The reason lies in focal or widespread unilateral or bilateral injury of the central thalamic and parathalamic nuclei and rostrocaudal regions. In some patients, an inconsistent but cognitively mediated response to external stimuli can be observed, which is reproducible or sustained long enough to be differentiated from reflexive behaviour; such condition is called a minimally conscious state. With the deep brain stimulation method, usually the region of centromedian-parafascicular complex in dominant hemisphere is stimulated with implantable electrodes. The method showed to be effective in rare cases of patients in vegetative state to improve awareness and responsiveness to external stimuli and ability to communicate, but no improvement of motoric control was observed. There was more remarkable improvement in the group of patients in the minimally conscious state, who were able of independent ambulation and reached some degree of independence in daily activities"},{"@xml:lang":"sl","#text":"Napredek na področju intenzivne medicine vedno več bolnikom omogoča, da preživijo stanje kome po težki možganski poškodbi. Pri nekaterih bolnikih po preživeli težki možganski poškodbi ali hipoksični možganski okvari ob izhodu iz stanja kome lahko opazimo redko, vendar skrajno obliko prizadetosti, ki se kaže kot stanje povrnjene zavesti, a brez zavedanja in hotenega odzivanja na dražljaje iz okolja - kar imenujemo vegetativno stanje (VS). Razlog za to je v žariščni, običajno obojestranski poškodbi talamičnih in subtalamičnih jeder ter rostrokavdalnega področja. Nekateri od teh bolnikov po določenem času pričnejo kazati različno intenzivne, vendar ponovljive in kognitivno pogojene odzive na zunanje dražljaje, ki jih je mogoče zanesljivo ločiti od refleksno pogojenih odzivov - kar imenujemo stanje minimalne odzivnosti. Metoda stimulacije globokih možganskih jeder (angl. deep brain stimulation), in sicer retikularne formacije in centromedianega-parafascikularnega kompleksa dominantne hemisfere, se je v redkih primerih izkazala za učinkovito pri izboljšanju odzivnosti bolnikov v vegetativnem stanju do te mere, da so se bolniki lahko smiselno sporazumevali in kazali, da navodila razumejo, niso pa se bili sposobni samostojno premikati.
