In doing so, it summarizes foreign experiences and develops its own knowledge and skills, first in the form of pharmaceutical care programs, and later in the form of pharmaceutical cognitive services (medication use review, advanced medication review). The new Pharmacy Act represents an appropriate solution for placing clinical pharmacy in the health system. Implementation of the law provisions is successful at all three levels of health care. Clinical pharmacists at clinical departments of hospitals participate in visits, carry out advanced medication reviews, run medication reconciliation and introduce zdravtherapeutic drug monitoring of selected medicines. In the pharmacies they perform medication use reviews, and in the health centers and elderly homes, they are established as pharmacists consultants. From year to year the number of specialists of clinical and community pharmacists increases. Clinical pharmacists are also involved in the performance of undergraduate and postgraduate courses in master's, doctoral and specialist studies, as well as in the therapeutic outcomes research and the publication of original and review scientific articles in the journals with an impact factor"},{"@xml:lang":"sl","#text":"Klinična farmacija se je v Sloveniji v zadnjih petnajstih letih uveljavila že v večini zdravstvenih institucij. Pri tem povzema tuje izkušnje in razvija lastna znanja in veščine, najprej v obliki programov farmacevtske skrbi, kasneje pa v obliki farmacevtskih kognitivnih storitev (pregled uporabe zdravil, farmakoterapijski pregled). Novi Zakon o lekarniški dejavnosti predstavlja ustrezno rešitev za umeščanje klinične farmacije v zdravstveni sistem. Implementacija določil zakona poteka uspešno na vseh treh nivojih zdravstvenega varstva. Klinični farmacevti na kliničnih oddelkih bolnišnic sodelujejo na vizitah, izvajajo farmakoterapijske preglede, usklajujejo zdravljenje z zdravili in uvajajo terapevtsko spremljanje plazemskih koncentracij izbranih zdravil. V lekarnah izvajajo preglede uporabe zdravil, v zdravstvenih domovih in domovih starejših občanov pa se uveljavljajo kot farmacevti svetovalci. Iz leta v leto se povečuje število specialistov in specializantov iz klinične in lekarniške farmacije.
