Seamless pharmaceutical care was systemically introduced in the Slovenian healthcare system with the >>Pharmacy practice act<< in 2016. However, the actual activities involved in the provision of seamless pharmaceutical care in hospital are defined in the recently published Rules on the provision of pharmacy services in hospital pharmacy that will come into force in 2023. Focusing on pa tients at high risk of medication errors, collaboration between healthcare professionals and development of appropriate information technology is supposed to contribute most to the effectiveness of seamless pharmaceutical care activities. The implementation of seamless care into clinical practice could be based on the conclusions of the research studies, performed in Slovenia. Moreover, it would be beneficial to evaluate the effectiveness of seamless pharmaceutical care model as it is expected to be implemented in the Slovenian healthcare system"},{"@xml:lang":"sl","#text":"Brezšivna skrb pomeni kontinuirano zagotavljanje varne in kakovostne oskrbe pacienta s strani vseh udeleženih zdravstvenih delavcev med ravnmi in znotraj ravni zdravstvenega varstva. V slovenski zdravstveni sistem smo brezšivno skrb pri zdravljenju z zdravili sistemsko uvedli z Zakonom o lekarniški dejavnosti v letu 2016. Nedavno objavljen Pravilnik o izvajanju storitev lekarniške dejavnosti v bolnišnični lekarni opredeljuje tudi vsebinske vidike brezšivne skrbi v bolnišnici, ki bodo stopili v veljavo leta 2023. K učinkovitosti izvajanja brezšivne skrbi naj bi najbolj prispevali usmerjenost na paciente z največjim tveganjem za napake pri zdravljenju z zdravili, sodelovanje zdravstvenih delavcev in razvoj ustrezne informacijske tehnologije. Pri implementaciji brezšivne skrbi v klinično prakso so lahko v pomoč rezultati že izvedenih raziskav v slovenskem prostoru.
