Despite the general techological development, less invasive surgical procedures and extensive postoperative rehabilitation, more than half of the patients do not return to preinjury levels of sports participation. Current rehabilitation practice after major knee operative procedures involves traditional approach to exercise, which mechanically stresses the musculoskeletal system. Such programmes include exercises for improving range of motion, gait re-education, weight-bearing exercise, training of the neuromuscular function and proprioception, as well as strength and endurance exercises, both performed voluntarily and by using electrical stimulation devices. Nevertheless, it seems that such programs are not sufficient in eliminating functional limitations after aforementioned surgical procedures. Therefore, it is necessary to explore new therapeutic tools that target the central nervous system or higher centers of motor control and include them in the existing rehabilitation practice. A variety of techniques for physical and mental activation have been developed for improving the physical funtion, when the movement execution is not possible (eg due to pain, impaired motor control and/or immobilisation). Among the most popular such cognitive techniques are motor imagery and action observation. This review decribes different cognitive strategies, which can potentially be implemented in the rehabilitation practice of the ortopedic patients</dc:description><dc:description xml:lang="sl">Operativni zdravljenji, kot sta totalna kolenska artroplastika in rekonstrukcija sprednjega križnega ligamenta, imata za posledico izraženo šibkost mišic iztegovalk kolena, ki lahko vztraja tudi nekaj let po operativnem posegu. Kljub splošnemu tehnološkemu napredku, manj invazivnim kirurškim posegom in obsežni pooperativni rehabilitaciji se več kot polovica pacientov ne vrne na raven športnega udejstvovanja pred posegom. Trenutna rehabilitacijska praksa po večjih operativnih posegih kolena obsega precej tradicionalen pristop k vadbi, ki mehansko obremenjuje mišično-skeletni sistem. Takšni vadbeni programi vključujejo vaje za povečanje obsega giba, ponovno učenje vzorca hoje, obremenjevanje in prenašanje teže, trening nevromišične funkcije ter vaje za moč in vzdržljivost, pri čemer se te izvajajo tako hoteno kot z uporabo električne stimulacije. Kljub temu se zdi, da takšni programi niso zadostni pri odpravljanju funkcionalnih omejitev po omenjenih operativnih posegih. Zato je treba raziskati nova terapevtska orodja, ki ciljajo na centralni živčni sistem oz. višje centre motorične kontrole, in jih vključiti v rehabilitacijsko prakso. Za izboljšanje telesne funkcije, ko želenega gibanja ni mogoče izvesti v celoti (npr. zaradi bolečine, oslabljene motorične kontrole in/ali imobilizacije sklepov), je bilo razvitih več tehnik za povečanje telesne in mentalne aktivacije brez izvajanja očitnega gibanja. Med priljubljenimi kognitivnimi tehnikami za izboljšanje telesne zmogljivosti sta gibalna predstava in opazovanje dejanj.
