The aim of our study was to establish the progress that patients had achieved in performing basic activities of daily living after rehabilitation at the Laško Thermal Spa Resort. We also wanted to find out whether or not we can predict the outcome of rehabilitation using the cognitive part of the Extended Barthel Index (EBI) assessment at admission, and whether the progress during rehabilitation is associated with the improvement of continence, patient age, type of stroke and duration of rehabilitation. Methods: At admission and at discharge, we evaluated 40 patients with after stroke who had been admitted for rehabilitation to the Laško Resort directly from the hospital. We collected data regarding patient age, gender, type of stroke (ischemic or haemorrhagic), side of paresis, time from stroke occurrence to the start of the rehabilitation, and length of rehabilitation (2 or 3 weeks). For assessing functional ability of the patients at admission and at discharge, weused the EBI (with the maximum possible score of 64 points). Results: The median age of patients was 76 years: 36 patients (90%) had suffered ischemic stroke and 4 patients (10%) haemorrhagic stroke. The length of rehabilitation was 3 weeks for 32 patients and 2 weeks for 8 patients. The median period from the occurrence of stroke to the admission to the spa (length of hospitalisation) was 23.5 days. The mean EBI score at admission and at discharge was 31.1 and 38.4, respectively. The total EBI score, cognitive EBI and physical EBI score was significantly higher at discharge. We did not find a correlation between the progression in physical EBI scores and either the time-period from the occurrence of stroke, the length of rehabilitation, or the type of stroke. We found that improvement in physical EBI scores was positively correlated to memory and vision EBI scores at admission but not with other cognitive EBI scores. We also found that improvement of continence was positively correlated to improvement in total EBI score. Conclusions: We found statistically significant improvements in functional performance in patients with stroke after 2 or 3 weeks of rehabilitation at the Laško Resort. The patients achieved more progress in terms of activities of daily living and less progress in terms of cognitive activities"},{"@xml:lang":"sl","#text":"Izhodišča: Možganska kap je glavni vzrok za dolgotrajne funkcijske omejitve bolnikov. Namen raziskave je bil ugotoviti, kakšen je napredek bolnikov po možganski kapi pri izvajanju osnovnih dnevnih aktivnosti po rehabilitaciji v zdravilišču Laško. Zanimalo nas je tudi, ali lahko iz ocen na kognitivnem delu razširjenega indeksa po Barthelovi (RBI) ob sprejemu sklepamo o uspšnosti funkcijskega okrevanja bolnikov in ali je napredovanje bolnikov na področju kontinence povezano z njihovo starostjo, tipom kapi, trajanjem in učinkovitostjo rehabilitacije. Metode: Ob sprejemu v zdravilišče in ob odpustu smo ocenili 40 bolnikov po možganski kapi, ki so jih napotili na rehabilitacijsko obravnavo v zdravilišče Laško neposredno iz bolnišnic. V raziskavo nismo vključili bolnikov, ki so smo jih ponovno sprejeli na obnovitveno rehabilitacijo. Zbrali smo podatke o starosti bolnikov, spolu, tipu kapi (ishemična ali hemoragična), strani prizadetosti, času od nastanka kapi do začetka rehabilitacije in o trajanju rehabilitacije (2 ali 3 tedne). Za oceno funkcijskih sposobnosti bolnikov ob sprejemu in odpustu smo uporabili lestvico RBI, na kateri je največje možno število točk 64. Rezultati: Mediana starosti bolnikov je bila 76 let. Ishemično možgansko kap je imelo 36 bolnikov (90 %), hemoragično pa štirje (10 %). Rehabilitacija je pri dvaintridesetih bolnikih (80 %) trajala tri tedne, pri osmih (20 %) pa dva. Mediana obdobja od nastanka kapi do sprejema bolnika v zdravilišče (čas trajanja bolnišnične obravnave) je bila 23,5 dni. Bolnike smo z lestvico RBI ocenili ob sprejemu na rehabilitacijsko obravnavo in ob odpustu. Povprečje ocen po skupnem RBI na začetku je bilo 31,1, ob odpustu pa 38,4. Ocene na skupnem RBI, telesnem in kognitivnem delu lestvice RBI so se pri bolnikih po rehabilitaciji statistično značilno izboljšale. Povezanosti med izboljšanjem ocen, doseženih na telesnem delu RBI, in starostjo, številom dni od nastanka kapi, časom trajanja rehabilitacije ali tipom kapi nismo ugotovili. Ugotovili smo, da je izboljšanje ocen na telesnem delu lestvice RBI povezano z višjimi ocenami, doseženimi na lestvici RBI na področju spomina in vida na začetku rehabilitacije, ni pa povezano z ocenami na drugih področjih kognitivnega dela lestvice RBI. Ugotovili smo tudi, da je napredek bolnikov na področju kontinence povezan z izboljšanjem ocen, doseženih na celotnem RBI. Zaključki: Pri bolnikih po možganski kapi smo po dva ali tritedenski rehabilitaciji v zdravilišču Laško ugotovili statistično značilno izboljšanje funkcijskega stanja v primerjavi z njihovim stanjem ob prihodu.
