Methods: We analysed the data from the rehabilitation processes of 82 youngsters with disabilities in transition from education to employment in the years 2009 and 2010 at the Vocational Rehabilitation Centre of the University Rehabilitation Institute in Ljubljana. In addition, a case is presented as an example of good practice has been presented. Results: The mean age of our clients was 28 years; they had mainly finished secondary school; they had been unemployed for 4.7 years on average; 45 % of them suffered from some form of psychopathology. The severity of functional limitation was assessed as moderate in 66 % of the cases. Three quarters of the youngsters decided to join the supporting programs in the vocational rehabilitation process. The example of good practice is a story about a young male college graduate with a mental disorder who succeeded in finding supported employment. Conclusions: The key point in influencing the transition outcome is that the vocational guidance and rehabilitation services should start as soon as possible. This is only possible when all the professionals identifying and guiding a youngster through different social contexts (health, education, employment) collaborate well</dc:description><dc:description xml:lang="sl">Izhodišča: V članku želimo z analizo obravnav mladih invalidov pri iskanju prve zaposlitve in s predstavitvijo primera dobre prakse opozoriti na kritične točke, s katerimi se pri svojem delu srečujemo člani tima zaposlitvene rehabilitacije v Centru za poklicno rehabilitacijo (CPR) na Univerzitetnem rehabilitacijskem inštitutu Republike Slovenije Soča. Metode: Analizirali smo podatke o obravnavi 82 oseb, ki so prvič iskale zaposlitev, v obdobju od leta 2009 do 2010 v CPR, v službah v Ljubljani in Mariboru. Predstavljamo tudi primer uspešne obravnave mladega moškega, ki je prvič iskal zaposlitev. Rezultati: Iskalci prve zaposlitve so bili v povprečju stari 28 let, največ jih je imelo V. stopnjo izobrazbe, pred obravnavo so bili povprečno 4,7 let brezposelni, 45 % jih je imelo duševne motnje. Ocenili smo, da so njihove težave srednje težke, in ugotovili, da se je kar tri četrtine obravnavanih oseb vključilo v zaposlitveno rehabilitacijo. Primer dobre prakse je zgodba o moškem z visoko stopnjo izobrazbe, ki je imel težave zaradi duševnih motenj in ki se je zaposlil v realnem delovnem okolju na podpornem delovnem mestu. Zaključek: Postopek usmerjanja v ustrezen poklic in pomoč pri prehodu na trg dela je težko izpeljati, saj je za to potrebno medresorsko in interdisciplinarno sodelovanje. Boljše uspehe lahko pričakujemo pri čim hitrejšem vključevanju mladih oseb v zaposlitveno rehabilitacijo.
