In the selected hospital wards medication reconciliation, preparation of personal medication card and counselling to patients regarding their pharmacotherapy after discharge were performed in more than 90% of patients, but only if the pharmacist was informed about the discharge. Despite the fact that the discharge therapy was optimised a week before the discharge, 126 pharmaceutical interventions were performed in 48/80 patients. The interventions were classified with PharmDISC system and they were most often (81.7%) performed due to the treatment costs or technical/formal problems. High values of selected clinical pharmacy key performance indicators confirm fairly consistent performance of pharmaceutical care at discharge from the wards, included in the study. Therefore, we suggest the extension and unification of the seamless care process at discharge also to other hospital wardsRetrospektivno smo analizirali farmacevtsko obravnavo bolnikov, ki so bili med 1. 9. 2018 in 30. 11. 2018 odpuščeni iz dveh oddelkov Univerzitetnega rehabilitacijskega inštituta Republike Slovenije % Soča. Usklajevanje zdravljenja z zdravili, izdelavo in predajo osebne kartice zdravil ter svetovanje bolnikom o odpustni terapiji z zdravili so na izbranih oddelkih izvedli pri več kot 90 % bolnikov, a le če je bil farmacevt obveščen o odpustu bolnika. Kljub optimizaciji odpustne terapije z zdravili v zadnjem tednu pred odpustom, je farmacevt ob odpustu pri 48 od 80 bolnikov z zdravili izvedel 126 farmacevtskih intervencij. Izvedene intervencije smo klasificirali po sistemu PharmDISC in v večini (81,7 %) so bile vezane na stroške zdravljenja ali tehnične/formalne težave. Visoke vrednosti izbranih kazalnikov kakovosti potrjujejo dokaj dosledno izvajanje farmacevtske obravnave v okviru brezšivne skrbi ob odpustu na izbranih oddelkih, zato predlagamo razširitev in poenotenje izvajanja brezšivne skrbi ob odpustu tudi na ostalih oddelkih inštitutaTEXTznanstveno časopisjejournalsSlovenian National E-content AggregatorNational and University Library of SloveniaSlovensko farmacevtsko društvo
