We hypothesized that supervised aerobic leg training improves endothelial function in the upper limb vessels. In our prospective study, 24 patients (3 women, 21 men, aged 50-80 years) with chronic heart failure were randomized toa control or a training group; which attended one-hour training sessions 3 times a week for 8 weeks. Before and after the training period, clinical and biochemical parameters, exercise capacity as well as endothelial dysfunction were determined. Endothelial dysfunction was assessed by high-resolution ultrasound measurements of brachial artery diameter and peak flow velocity at rest, after reactive hyperemia (causing flow mediated endothelium-dependent dilation), and after gylceriltrinitrate administration (causing endothelium-independent vasodilatation). Povezana naj bi bila s spremembami obrobnih tkiv, med katerimi je zelo pomexnbna endotelijska disfunkcija, tj. okrnjena sposobnost endotelija sprožiti razširitev žil pod vplivom fizioloških spodbud:Domnevali smo, da nadzorovana aerobna telesna vadba spodnjih udov pri bohikih s kroničnini popuščanjem srca izboljša endotelijsko funkcijo na žilju nadlakt: V prospektivni raziskavi smo 24 bolnikov (3 ženske, 21 moških, starih50-80 let) s kroničnim popuščanjem srca naključnostno razporedili v skupino, ki je 8 tednov nadzorovano vadila 3-krat na teden po eno uro in kontrolno skupino, ki ni trenirala. Pred obdobjem telesne vadbe in po njem smopri vseh preiskovancih določili klinične značilnosti in ravni krvnih maščob, izmerili obremenitveno zmogljivost ter opredelili endotelijsko disfunkcijo z merjenjem od endotelija odvisne vazodilatacije brahialne arterije z ultrazvokom smo določili premer brahialne arterije in hitrost pretoka krvi v njej med mirovanjem, med povečanjem pretoka (ki povzroča od endotelija odvisno vazodilatacijo) ter po zaužitju gliceril trinitrata (ki povzroča od endotelija neodvisno vazodilatacijo).
