Primary open angle glaucoma (POAG) is a syndrome of progressive optic neuropathy characterized by optic nerve head excavation and visual fielddefects. Poor correlation between IOP and progression of glaucoma diseasesets vascular mechanism in the centre of attention. By Color Doppler sonography, quantification of blood flow changes in vessels, which supply optic nerve head, is possible. We wanted to find out whether there are changesin the circulation of central retinal artery and posterior ciliary arteries in patients with primary open angle glaucoma treated with selective or nonselective beta-blockers. Methods. 44 patients (88 eyes) were divided into two groups: group 1: 22 patients (44 eyes) treated with selective beta-blockers (Betaxolol 0.5%) and group 2: 22 patients (44 eyes) treated withnonselective beta-blockers (Timolol 0.5%). Vascular indices (RI, PI) were measured in the central retinal artery and posterior ciliary arteries. Results. We found decreased blood flow and increased vascular indices in both groups of patients, statistically significant difference between group 1 and group 2: blood flow velocity was higher and vascular indices were lower in group 1 (Betaxolol 0.5%) compared to group 2 (Timolol 0..5%). Conclusions. Selective beta-blockers (calcium channel blockers) act more vasoactively and neuroprotectively comparing to nonselective beta-blockers</dc:description><dc:description xml:lang="sl">Izhodišča. Primarni glavkom odprtega zakotja je sindrom progresivne optične nevropatije, za katero je značilna ekskavacija papile vidnega živca in izpadi v vidnem polju. Slaba korelacija med očesnim tlakom in napredovanjem glavkomske bolezni postavlja vaskularni mehanizem v središče pozornosti. Z barvno doplersko ultrazvočno preiskavo lahko kvantitativno določimo spremembe krvnega pretoka v področju papile vidnega živca. Želeli smo ugotoviti, ali obstajajo razlike v hitrosti pretoka krvi in uporu v centralni retinalni arteriji in posteriornih ciliarnih arterijah pri bolnikih s primarnim glavkomom odprtega zakotja, ki so jemali očesne kapljice s selektivnimi oziroma z neselektivnimi beta-blokatorji. Metode. 44 bolnikov (88 oči) smo razdelili v dve skupini. Prva skupina: 22 bolnikov (44 oči), ki so jemali selektivne beta-blokatorje (Betaxolol 0,5%), druga skupina: 22 bolnikov (44 oči), ki so bili zdravljeni z neselektivnimi beta-blokatorji (Timolol 0,5%). Žilni upori (RI, PI) so bili izmerjeni v centralni retinalni arteriji in v posteriornih ciliarnih arterijah. Rezultati. Ugotovili smo zmanjšanje hitrostikrvnega pretoka in povečanje žilnega upora v obeh skupinah pacientov, statistično značilno razliko med prvo skupino in drugo skupino: hitrost krvnega pretoka je bila večja, žilni upor pa manjši v prvi skupini (Betaxolol 0,5%) v primerjavi z drugo skupino (Timolol 0,5%). Zaključki.
