In a selected group of cases, SK can successfully replace more aggressive surgical procedures. Methods A retrospective analysis of all intrapleural SK applications at our department between 2004 and 2007 was performed. Results 14patients met the study inclusion criteria: 7 parapneumonic effusions, 5 residual hemothoraces and 2 loculated malignant effusions. Significant increase of the drained volume with notable radiological and clinical improvement followed the applications of 500,000 IU of SK in all patients and 9 patients (64%) were spared the painful thoracotomy. Conclusions Our data supports the evidence on high efficiency of the intrapleural fibrinolysis withSK in a selected population of complicated pleural effusions"},{"@xml:lang":"sl","#text":"Izhodišča Intraplevralna fibrinoliza s streptokinazo (SK) je stara in že nekoliko pozabljena metoda zdravljenja zanemarjenih plevralnih izlivov, ki lahko v nekaterih primerih zelo učinkovito nadomesti precej bolj agresivno kirurško zdravljenje. Metode Retrospektivno smo obdelali podatke o bolnikih, ki smo jih na Kliničnem oddelku za torakalno kirurgijo v letih 2004-2007 zdravili z intraplevralnim dajanjem SK. Rezultati V tem času smo SK uporabili pri 19 bolnikih, med njimi jih je 14 izpolnjevalo merila za vključitev v raziskavo: 7 bolnikov s parapnevmoničnim izlivom, 5 bolnikov z ostankom hemotoraksa in 2 bolnika s septiranim malignim izlivom. Pri vseh 14 bolnikih smo s trikratnim zaporednim dajanjem 500.000IE SK dosegli opazno povečanje izločanja po drenu ter pomembno radiološko in klinično izboljšanje, zaradi katerega smo se lahko pri 9 bolnikih (64%) izognili torakotomiji.
