The hypothesis that induced sputum cell counts and mediators could help discriminate between between asthma and chronic obstructive pulmonary disease was tested. Cell counts and the concentration ofECP and PMN elastase were measured in induced sputum and serum of 21 healthysubjects,17 patients with asthma and 6 with stable chronic obstructive pulmonary disease. Induced sputum eosinophils were significantly higher in case of asthma (8.2 10.8%) than in healthy subjects (0.3 0.6%) (p &lt; 0.05),but chronic obstructive pulmonary disease (2.4 1.9). Neutrophil counts were significantly higher in patients with chronic obstructive pulmonary disease (89.5%, S. D. 2.0%) than in those with asthma (65.8%, S. D. 23.7%) (p &lt; 0.05). Induced sputum ECP were higher in asthma (148 104mig/1) and chronicobstructive pulmonary disease (113 122mig/1) than in healthy subjects (48 37mig/1) (p &lt; 0.05). Eosinophil counts and ECP concentration in induced sputum could be used to discriminate between patients with asthma and healthy subjects. Neutrophil counts in induced sputum could be used to discriminate between patients with chronic obstructive pulmonary disease and asthma. However, measurement of ECP and PMN elastase concentration in induced sputum and serum would not discriminate between patients with chronic obstructive pulmonary disease and asthma</dc:description><dc:description xml:lang="sl">Vnetje v dihalnih poteh ima pomembno vlogo pri nastanku obstruktivne pljučne bolezni. S kliničnimi preiskavami prepoznavamo le spremembe delovanja, težko pa odkrijemo in ločimo vrsto vnetja. Izzvan izmeček (induciran sputum) je novejša, neinvazivna metoda, s katero ugotavljamo vrsto in aktivnost vnetnega dogajanja v dihalnih poteh. Želeli smo ugotoviti ali je izbrana metoda diagnostično uporabna za ugotavljanje vrste in aktivnosti vnetnega dogajanja pri bolnikih z astmo in kronično obstruktivno pljučno boleznijo. Zanimal nas je delež vnetnih celic in koncentracija topnih označevalcev vnetja. V raziskavo smo vključili 21 zdravih preiskovancev, 17 bolnikov z astmo in 6 bolnikov s kronično obstruktivno pljučno boleznijo v ustaljeni fazi bolezni. Zmetodo izzivanja izmečka (induciranja sputuma) smo pridobili vzorce in v njihdoločili delež, vrsto vnetnih celic in koncentracijo eozinofilne kationskebeljakovine in nevtrofilne elastaze. Merili smo tudi serumsko koncentracijo eozinofilne kationske beljakovine in nevtrofilne elastaze. Deležeozinofilcev je bil pri bolnikih z astmo (8,2+-10,8%) statistično značilno večji kot pri zdravih preiskovancih (0,3 t 0,6 %) (p &lt; 0,05). Delež eozinofilcev pri bolnikih s KOPB je bil (2,4+- 1,9%). Delež nevtrofilcev je bil pri bolnikih s kronično obstruktivno pljučno boleznijo (89,5+- 2,0 %) statistično značilno večji kot pri bolnikih z astmo (65,8+-23,7%) (p&lt;0,05). Koncentracija eozinofilne kationske beljakovine v izzvanem izmečku je bila pribolnikih z astmo (148+-103,5mig /1) in pri bolnikih s kronično obstruktivnopljučno boleznijo (112,7+-122,2mig/l) statistično značilno višja kot pri zdravih preiskovancih (48,3+-36,8mig/l) (p&lt;00,05). Delež eozinofilcev in koncentracija eozinofilne kationske beljakovine v izzvanem izmečku sta diagnostično uporabna za razlikovanje bolnikov z astmo od zdrave populacije.
