Because of nonspecific symptoms such as abdominal and back pain, the diagnosis is difficult to make, particulary in patients with no previous history of aneurysm. We report on a patient with a ruptured abdominal aortic aneurysm andmassive retro and paraaortic haematoma, who developed a rare complication fistula communicating with the inferior vena cava. The patient, who was in haemorrhagic shock, requierd an emergency operation. Začetni znaki, kot so bolečine v trebuhu in križu, so neznačilni in predstavljajo diagnostični problem predvsem pri bolnikih, ki anevrizme še nimajo ugotovljene. Opisan je primer bolnika, pri katerem je prišlo do razpokeanevrizme abdominalne aorte z obsežnim retro in paraaortnim hematomom ter do redkega zapleta, fistule v spodnjo veno kavo, ki je bila ugotovljena z aortografijo.
