Compared with standard 20-gauge pars plana vitrectomy, the smaller25-gauge system allows for self-sealing transconjunctival pars plana sclerotomies. This leads to decreased surgical time, less postoperative inflamation, and faster postoperative recovery. Ideally it is used for vitrectomy for macular hole, macular pucker, vitreous haemorrhage and macular edema. In this article we describe historical moments in development of this technique, benefits, possible complications and experience at our clinic.
