Its prevalence ranges from 6.9 to 10%. It is treated with drugs and other non-pharmacological therapies. Second-line treatment include the 8 % capsaicin patch, where capsaicin binds to the TRPV1 receptor of the primary sensory neuron and results in the defunctionalization and degeneration of sensory nerve endings, thereby reducing pain. Methods: In a healthy 51-year old patient two years after caustic-allergic skin and nerve injury in the area of both ankles and calfs and surgical care with skin grafts, burning pain with intermittent electrical shock persisted, despite high doses of gabapentin. At night, the pain was exacerbated, with insomnia and consequently fatigue and poor mood. Pregabalin and duloxetine were ineffective, lidocaine patch also did not reduce the pain. We therefore applied to the skin graft an 8 % capsaicin patch. Results: After a capsaicin patch application to the skin graft, a three- to four-week delayed effect (less burning, tingling, less pain and better sleep) occurred each time. The patient reported a prolonged therapeutic effect between applications, increasing from three months after the first application to seven months after the second and 10 months after the third application. Conclusion: Treatment of persistent and severe chronic neuropathic pain with an 8 % capsaicin patch is likely to be effective and safe in skin grafts when the wound is already healed. However, more clinical data will be needed to fully evaluate the safety and efficacy of capsaicin patch use on skin grafts"},{"@xml:lang":"sl","#text":"Uvod: Nevropatska bolečina je posledica okvare ali bolezni somatosenzoričnega živčnega sistema. Njena prevalenca znaša od 6,9 % do 10 %. Zdravimo jo z zdravili ter drugimi nefarmakološkimi ukrepi. Med zdravila druge izbire sodi tudi 8-odstotni kapsaicinski obliž. Pri tem se kapsaicin veže na TRPV1 receptor primarnega senzoričnega nevrona in povzroči prenehanje delovanja senzoričnih živčnih končičev in njihovo degeneracijo, s tem pa zmanjšanje bolečine. Metode: Pri sicer zdravem 51-letnem bolniku, dve leti po kavstično-alergični poškodbi kože in živcev v predelu obeh gležnjev in meč ter kirurški oskrbi s kožnimi presadki, je kljub visokim odmerkom gabapentina vztrajala pekoča bolečina z občasnimi električnimi sunki. Ponoči so se bolečine še okrepile, zato je imel bolnik težave zaradi nespečnosti s posledično utrujenostjo in slabim razpoloženjem. Pregabalin in duloksetin sta bila neučinkovita, tudi lidokainski obliž ni zmanjšal bolečin. Zato smo se odločili, da na kožni presadek namestimo 8-odstotni kapsaicinski obliž. Rezultati: Po vsakokratni uporabi kapsaicinskega obliža na kožnem presadku se je pojavila tri do štiri-tedenska zakasnitev oz. odloženo učinkovanje (manj izražen pekoč občutek, mravljinčenje, manjša bolečina in boljši spanec). Bolnik je poročal o vse daljšem terapevtskem učinku med posameznimi aplikacijami, ki se je povečal s treh mesecev po prvi aplikaciji na sedem mesecev po drugi in 10 mesecev po tretji aplikaciji. Zaključek: Zdravljenje trdovratne in hude kronične nevropatske bolečine z 8-odstotnim kapsaicinskim obližem je verjetno učinkovito in varno pri kožnih presadkih, ko je rana že zaceljena.
